Quick answer
PHIN’s September 2026 market update shows 247,495 private admissions across the UK in the first quarter of 2026, slightly above the previous record. Self-pay admissions reached their highest ever level and a 31% share, while insured admissions fell 2.6%, the first fall in a first quarter in five years. Cataract surgery led the growth, rising 12.8% to 22,805 admissions. For private clinics, the shift towards patients paying directly rewards capacity, pricing clarity and the right equipment, all of which usually need investment ahead of the income.
What PHIN’s data shows
The Private Healthcare Information Network’s UK market update, last updated on 8 September 2026, covers January to March 2026. Total private admissions were 247,495, up less than 1% on the same quarter of 2025, which had been the highest on record. Insurance funded 69% of admissions and self-pay 31%.
Insured admissions fell 2.6%, or 4,530 admissions, the first first-quarter fall in five years. Self-pay admissions rose to their highest ever level and increased in every English region. A record 11,600 consultants were actively practising privately, the highest of any quarter in the past five years.
Cataracts lead, and older patients pay directly
Cataract surgery was the biggest riser, up 12.8% to 22,805 admissions. PHIN’s press release says self-pay cataract admissions rose 21% and accounted for 46% of the UK increase in self-pay admissions, with growth among patients in their 60s, 70s and 80s. Eyelid surgery and other elective procedures also grew, while some endoscopy volumes fell.
PHIN cautioned that the growth should not be seen as uniform across the market, and said it expects the next quarter to show stronger growth than the same quarter of 2025. Its figures exclude outpatient diagnostics, physiotherapy and mental health services, so clinics in those areas should read them as a guide to patient behaviour rather than a direct measure of their market.
The age profile matters for planning. PHIN reports the largest increases in admissions among patients in their 60s and 70s, the groups most likely to need cataract, joint and other elective procedures. Clinics serving an older local population, or with links to consultants in those specialties, are the most likely to see demand continue to build.
What it means for private clinics
Self-pay patients compare on price, waiting time and convenience, so capacity and equipment matter. Clinics adding ophthalmic, diagnostic or day-case work often need imaging and treatment equipment, which equipment finance spreads over its working life. New or refitted procedure rooms, recovery areas and consulting suites fall under refurbishment and fit-out finance.
With more consultants practising privately, clinics that can offer rooms, theatre time and support staff are well placed to attract them. Recruitment finance covers nurses and support teams, while premises finance and expansion finance cover buying a building or opening a second site.
Lenders considering a clinic usually look at the mix of insured and self-pay income, consultant agreements, CQC registration and the track record of the services being expanded. A clear plan showing how new equipment or rooms will be used, and how quickly they will pay their way, helps an application move faster.
Practical steps for clinic owners
- Review which procedures your patients pay for directly, and where demand outstrips capacity.
- Publish clear self-pay prices and packages.
- Cost the equipment and space needed to add high-demand procedures.
- Plan funding before committing to a lease or fit-out.
